Don’t give out your social security number, not even the last four digits. Make the requester convince you that they absolutely need it and why. You’re only legally required to provide your social security number to banking institutions, employers and other companies required to report to the IRS.
Medical providers and other service providers who routinely ask for your SS# just don’t need and you should not provide it!
